Kindergarten Registration Information
2010-2011
For a child to be considered registered in our kindergarten program,
we require the following forms to be on file:
| 1) Kindergarten Registration Form 2) Birth Certificate 3) Current certification of immunization & kindergarten physical 4) Current dental record 5) Field Trip authorization form 6) EFT Form 7) All DCFS Required Registration Forms 8) $75 book fee (Kindergarten only) |
Enrollment for Fall 2009 is considered on a first come – first served basis.
The first 19 students fully registered and paid, for each class will be served
Due to the limited number of spaces and high demand for quality programming;
The first month’s tuition will be payable by no later than August 1st 2009
· Subsequent payments can be monthly or weekly.
